As noted in Part 1 of this two-part review, school attendance is an important foundational competency for children and adolescents, and school absenteeism has been linked to myriad short- and long-term negative consequences, even into adulthood. Categorical and dimensional approaches for this population have been developed. This article (Part 2 of a two-part review) discusses compatibilities of categorical and dimensional approaches for school attendance and school absenteeism and how these approaches can inform one another. The article also poses a multidimensional (...) multi-tiered system of supports pyramid model as a mechanism for reconciling these approaches, promoting school attendance (and/or prevention of school absenteeism), establishing early warning systems for nimble response to school attendance problems, assisting with global policy review and dissemination and implementation, and adapting to future changes in education and technology. Throughout this paper PJ Kearney attempts to balance the risks and benefits of different approaches in paediatric oncology. Decisions have to be considered both in the short and the long term. Where religious beliefs, such as those held by Jehovah's Witnesses in relation to blood transfusions, conflict with normal medical practice the decision is often removed from the doctor, parents or patient to the courts. This sort of solution can be counter-productive, especially as good health care and subsequent recovery (...) rely, to a large extent, on good relationships between and among the parties concerned. Destruction of these relationships, for whatever reason, often has a detrimental effect on the patient, in whose best interest everyone believes they are acting. (shrink)

Autopathographies are an expanding genre of books and articles that are a potential resource for students interested in the medical humanities. New curricula emphasise the need to familiarise medical students with the patient’s point of view. Different specialities compete for the student’s attention and the medical humanities are not an exception. Some form of assessment is necessary to reflect the importance of the patient’s perspective. One way may be to request students analyse their chosen autopathography. The article presents an example (...) based on one of the better know accounts. The identified themes reflect the sudden disruption in the author’s biography. The topics are connected to the medical disorder and illustrated by quotations. The different categories of autopathography can be helpful in understanding the author’s perspective. In this instance the author was a full time editor before his stroke. The style of the book employs professional distance from the narrative. This is more compatible with the description of metapathography rather than the original classification of autopathogrphies as religious, angry or alternative. (shrink)

BackgroundReflexive pitch perturbation experiments are commonly used to investigate the neural mechanisms underlying vocal motor control. In these experiments, the fundamental frequency–the acoustic correlate of pitch–of a speech signal is shifted unexpectedly and played back to the speaker via headphones in near real-time. In response to the shift, speakers increase or decrease their fundamental frequency in the direction opposing the shift so that their perceived pitch is closer to what they intended. The goal of the current work is to develop (...) a quantitative model of responses to reflexive perturbations that can be interpreted in terms of the physiological mechanisms underlying the response and that captures both group-mean data and individual subject responses.MethodsA model framework was established that allowed the specification of several models based on Proportional-Integral-Derivative and State-Space/Directions Into Velocities of Articulators (DIVA) model classes. The performance of 19 models was compared in fitting experimental data from two published studies. The models were evaluated in terms of their ability to capture both population-level responses and individual differences in sensorimotor control processes.ResultsA three-parameter DIVA model performed best when fitting group-mean data from both studies; this model is equivalent to a single-rate state-space model and a first-order low pass filter model. The same model also provided stable estimates of parameters across samples from individual subject data and performed among the best models to differentiate between subjects. The three parameters correspond to gains in the auditory feedback controller’s response to a perceived error, the delay of this response, and the gain of the somatosensory feedback controller’s “resistance” to this correction. Excellent fits were also obtained from a four-parameter model with an additional auditory velocity error term; this model was better able to capture multi-component reflexive responses seen in some individual subjects.ConclusionOur results demonstrate the stereotyped nature of an individual’s responses to pitch perturbations. Further, we identified a model that captures population responses to pitch perturbations and characterizes individual differences in a stable manner with parameters that relate to underlying motor control capabilities. Future work will evaluate the model in characterizing responses from individuals with communication disorders. (shrink)

Descartes's dualism did not result from Cartesian doubts, Christian beliefs, from a bias against animal nature, or from a conflict of reason and emotion. In fact, Descartes's dualism was the very fruitful product of the mechanistic conception of causality with which the French philosopher sought to replace the souls, qualities and intelligences contemporaries put forward as alternatives for the outdated Aristotelian principles of matter, form and privation. Descartes's naturalistic turn in physiology and physics not only formed the basis for his (...) dualistic metaphysics; it still bears fruit in the work of scientists such as Antonio R. Damasio today. Basado enteramente en pruebas internas, este estudio se propone mostrar que el manuscrito único del Dīwān de Ibn Quzmān, publicado en edición facsímil por David de Gunzburg en 1896, es una copia dictada. Se indica también que el copista, a quien con frecuencia se ha culpado de introducir en el texto popular andaluz correcciones o clasicismos, no estaba bien equipado para desempeñar tal papel. Si bien era un calígrafo de primera categoría, se aducen pruebas de que su conocimiento del árabe (...) no estaba a la altura de la calidad de su caligrafía. El estudio muestra que él fue el responsable de una amplia gama de problemas textuales que ha dejado perplejos a sucesivos editores del Dīwān. En marcado contraste, se muestra que Ibn Quzmān poseía un dominio magistral de la lengua árabe y de las tradiciones-árabes clásicas. Al parecer también había adquirido un alto grado de 'aculturación' en la lengua romance. Además de ofrecer las pruebas en que se basan estos asertos, el estudio constituye un ejemplo ilustrativo de cómo estas pruebas pueden ayudar a una mejor lectura e interpretación del Dīwān de Ibn Quzmān. At least since the seminal work of the (US) National Commission for the Protection of Human Subjects of Biomedical and Behavioral Research in the 1970s, a fundamental distinction between research and practice has underwritten both conceptual work in research ethics and regulations governing research involving human subjects. Notwithstanding its undoubted historical importance, I believe the distinction is problematic because it misrepresents clinical inquiry. In this essay, I aim to clarify the character of clinical inquiry by identifying crucial contextual constraints on (...) justification constitutive of clinical science. This analysis shows that, from an epistemological point of view, clinical research and clinical practice are not sharply distinct but intimately intertwined. This result is important in its own right—an enriched understanding of the epistemology of clinical research is valuable in and of itself. But this result is also important because it has profound implications for the ethics of human subjects research. (shrink)

In 'Vague Identity: Evans Misunderstood' David Lewis defends Gareth Evans against a widespread misunderstanding of an argument that appeared in his article 'Can There be Vague Objects?'. Lewis takes himself to be 'defending Evans' and not just correcting a mistake; witness his remark that, 'As misunderstood, Evans is a pitiful figure: a "technical philosopher" out of control of his technicalities, taken in by a fallacious proof of an absurd conclusion'. Let me say at the outset that I take Lewis to (...) be exactly right in regarding as a misunderstanding the interpretation of Evans which he exposes as such. On Lewis's account, everything Evans says 'falls into place', and he quotes from a letter that 'settles the matter'. (shrink)

This commentary explores biological models of analogical and associative learning in support of Illusion 1 and Illusion 4 in D. Noble’s target article. The intent is to support Noble’s theses of emergent higher level functionality from lower level stochastic dynamics and his etiological claim that “there is no privileged level of causation” through a biosemiotic lens. Upon these arguments, a case for creative agency via higher-dimensional constraints will also be made in support of Noble’s claim that organismic behavior is actively (...) directed by an agent. Dr Gray leaves us with a question at the conclusion of his article--how should we choose priorities? He says that the debate so far has been mainly on what we should choose, but perhaps we should consider how to choose even more. Under the various subheadings of Criteria, Principles and Persons Dr Gray sets out the pros and cons of the arguments in the priority debates and tries to offer some more specific guidelines to offset the criticism that the government's (...) priority discussions have been too generalised. Lysias, describing how the Thirty were established in the government of Athens, begins with the sentence ναστς δ θηραμνης κλευσεν ὑμς τρι$κοντα νδράσιν πιτρΨαι τν πóλιν τῇ πολιτεᾳ χρσθαι ν Δρακοντδης πφαινεν Commenting on the last clause the judicious Thirlwall observes that ‘the precise meaning of these words is very doubtful. There is almost equal difficulty, whether we suppose that they refer to a proposition then made, or to one which was to be made, by Dracontides.’ Thirlwall has not expressed (...) his meaning as precisely as Lysias; the uncertainty lies, not in the words, nor in the reference intended by Lysias, but in the mind of the historian, who is conscious that, whether he refers the proposition of Dracontides to the occasion indicated by Lysias or to another, he will encounter almost equal difficulty. The difficulty however, arising from the apparent inconsistencies in the evidence of the ancient authorities on the date of the appointment of the Thirty, is not Thirlwall's only, and after a century of discussion it still vexes every student of the period. In this much-vexed passage a form or variant of ναγνρισς is described and exemplified. It is said to be συνθετ, which word all the translations I have seen render ‘compound’ or ‘composite,’ but their authors either omit or clearly fail to explain in what sense this form is said to be ‘composite.’ I believe that this translation is wrong, and that the word here means something else, συνθετος λόγος is good Greek for ‘a made-up tale’ . The point is not (...) that what is said or told is necessarily a lie, but that it is untrue, baseless, not founded on fact. All this is supported by the use of the corresponding verb in e.g. Thuc. I. cc. XXI. and XCVII., where it is opposed to the truthful recording of facts . Further, it justifies Dacier's reading in 1459a 21, where the relative clause refers to στοραις not to τς συνθσεις. The characteristic of a poetic σνθεσις is that it is a connected narrative which has a causal nexus between its parts, and so is of μα πρξις, whereas an στορα is a record of disconnected happenings to a single person or group of persons during a certain period. Such a σνθις may be merely made-up or fictional. It is specially so where it has no basis in fact , but is the sheer invention of the poet. Clearly such a fiction must be rendered acceptable or plausible, and this can only be done through someone being misled, and this misleading implies that that someone draws a wrong conclusion from evidence present or presented to him— i.e. it presupposes a παραλογισμός. The author deals with the meaning of metamerism. Two contrary points of view may serve to clear the problem. From the morphological point of view probably outgrowth of the organism in longitudinal direction has led to the development of metameres. So the first segmentation is due to a functional differentiation. In Phylogeny this segmentation has been maintained and the originally developed metameres remain the primary morphotic units of the organisms. Further specialising of function and organ-concentration destroyed the original absolute metamerism (...) and only left a rest of the original.The functional point of view renounces an original absolute metamerism for the present moment. It only asks for the functional meaning of the segmentation in organisms, showing this segmentation. From the functional point of view it is possible that metamerism may be the morphological expression of quite different functional requirements and that in one organism the metamerism starts from another system of organs than in another.Es wird gefragt welche Bedeutung die Metamerie haben kann. Zur Erläuterung des Problems werden zwei extremen Betrachtungsweisen einander gegenübergestellt. Sie fragt bloss nach der funktionellen Bedeutung, welche die Metamerie besitzt bei denjenigen Organismen, wo diese gefunden wird. Sie achtet es möglich, dass Metamerie der Ausdruck sein kann von ganz verschiedenen funktionellen Anforderungen und dass somit die Metamerie bei der einen Tiergruppe von einem ganz anderen Organsystem ausgeht wei bei einer anderen.
